Rates for Services
Dr. Marr is an Out of Network/Fee for Service provider, meaning she is not in network with any insurance carrier at this practice location. She will collect payment at the time services are rendered.
If you want to access your Out of Network benefits for services, you will still pay at the time services are rendered. Dr. Marr can then provide a “Super Bill” to you upon your request. This is a receipt to send into your insurance carrier and they will send you a reimbursement based upon your Out of Network benefits. Each insurance carrier has a different method for this process, sometimes including a form that you must fill out. Please be sure to contact your insurance and learn what the process will be for you prior to your visit.

Notification of Federal Protections Against Surprise Medical Bills
As of January 2022, there are new federal protections for private pay and out of network billing. This is a notice of those federal protections. Given that Natalie M. Marr, Psy.D., LP Counseling (dba of SBTV LLC) is a fee for service provider, these protections afford you the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who will not be using insurance an estimate of the bill for medical items and services. This protects you from “Surprise Billing” defined as an unexpected balance billing. The following outline your protections under these new federal guidelines:
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. Natalie M. Marr, Psy.D., Counseling (dba of SBTV LLC) provides this estimate within the documentation of your initial informed consent for treatment, titled “Service Rates and Your Rights and Protections Against Surprise Medical Bills”. This information is signed by you, the consumer, and kept on file for the duration of your treatment at Natalie M. Marr, Psy.D., LP Counseling (dba of SBTV LLC). It will be updated each year at the top of the year and resigned, by you the consumer. In addition, anytime there is a change in service rates, you will be given 30 days or more notice of those rate changes.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
